g) description of your ailment (head/neck/upper extremities/thoracic/lower extremities/lumbar) and pain scale (1- 10)
h) choice of motivation (get rid of pain / inner peace / more confidence / look nice / feel better / strengthen spine)

j) delivery address (only if device is ordered)

If for some reason you do not provide this personal data, we will not be able to conclude a contract with you and you will not be able to use our services.
